Job Title: RN Delegating Nurse/Case Manager (DN/CM)

POSITION SUMMARY Under the direction of the Director of Assisted Living/DN the DN/CM will authorize the unlicensed and licensed associate to perform nursing tasks while retaining full accountability. This will be accomplished by assuring each associate has proper training, ongoing supervision and evaluation of performance. The DN/CM will be available 24/7 for medical consultation.

 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

 

1. Within 14 days of a new resident’s admission the DN/CM will: assess the resident, review the resident’s current medical profile, including all prescription and non-prescription medications and make appropriate recommendations to the physician and or ALU Coordinator for follow up. The DN/CM will assess the medication technician’s ability to administer medications to the resident, or the resident’s ability to self-administer medications.

 

2. At least every 90 days re-assess residents and the medications of all residents who are capable of self-administering, or who self administer with reminders, supervision, or physical assistance to determine their continued ability to self-medicate.

 

3. Perform on-site reviews every 45 days for all residents who require that medications be administered by non-licensed personnel. This review shall include review of the Physicians’ Order Sheet, the Medication Administration Record (MAR), an assessment of the resident and the non-licensed person’s continued ability to administer medications.

 

4. Responsible for developing resident plan of care/service plan and updates. Provide coordination and consultation for other services as needed. Review resident records for compliance with all state regulations. Complete all federal/state requirements through assessments of all residents. Complete and review levels of care for residents.

 

5. Teach and/or coordinate the teaching of the approved 20 hour Maryland Board of Nursing Medication Technicians Training Program for Assisted Living to employees who qualify for the training.

6. As needed teach the approved 4 hour Maryland Board of Nursing Mediation Technician Clinical update to all trained Medication Technicians every two years.

 

7. Conduct in-services as appropriate.

 

  1. Will assist in maintain a tracking system to ensure that all Medications Technicians certification are current and in good standing.

 

9. The DM/CM will make recommendations to the Director of Assisted Living/DN and AL Coordinator regarding withholding the privilege of a Medication Technician in administering medications if the DM/CM believes the Medication Technician cannot administer medication in a safe manner.

 

10. Be available to staff members to answer questions concerning resident medical issues and/or medication issues.

 

11. Consult with ALU Coordinator, Administrator, families, physicians and pharmacy personnel as needed and/or as requested.

 

12. Ensure that all record keeping conforms to requirements set forth by Federal, State, and Local Agencies.

 

13. Support the Director of AL/DN and ALU Coordinator with scheduling, budget, payroll and on-call scheduling responsibilities as needed.

 

14. Update the Resident Assessment Tool every six (6) months with change in condition.

 

CHARTING AND DOCUMENTATION

 

1. Complete and file required forms upon resident's admission, transfer and discharge.

 

2. Ensure that attending physician's review, record and sign their progress notes and orders in accordance with facility policies.

 

3. Receive telephone orders from physicians and record accurately.

 

4. Transcribe physician's orders to resident's charts, MAR, TAR, ADL and Lab books accurately.

 

5. Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident as well as the resident's response to care. Review previous nurse’s notes to assure continuity and follow up.

 

6. Date, time and sign all entries in the clinical record.

 

7. Write legibly in blue or black ink.

 

8. Use only authorized abbreviations established by this facility when recording information.

 

9. Chart all reports of accidents, incidents and change of condition.

 

10. Complete incident report for all accidents and injuries - witnessed or un-witnessed; initiate un-witnessed injury investigation when indicated.

 

11. Record new or changed diet orders and forward information to Dietary Department.

 

12. Report all discrepancies noted concerning physician's orders, diet changes, charting errors to Director of AL/DN and/or ALU Coordinator. Complete Medication error reports as indicated.

 

13. Document medication and treatment administration and refusal and response to drugs per facility policy. Report resident’s refusal of medications to physician.

 

 

SAFETY AND SANITATION

 

1. Observe and follow established infection control policies and procedures; ensure that staff follows procedures and that par levels of personal protective devices is available.

2. Ensure that Assisted Living units (resident rooms, commons areas, etc.) are maintained in a clean, safe and sanitary manner.

3. Participate in all fire safety and disaster preparedness drills in a safe and professional manner.

4. Follow established safety regulations for the use of equipment and supplies and ensure that staff follow established polices for their use.

5. Report broken or damaged equipment and supplies and complete work order slips as indicated.

6. Institute appropriate infection control procedures when residents require specific precautions and/or isolation.

 

 

PERSONAL AND PROFESSIONAL

 

1. Follow all rules and policies as outlined by regulatory agencies, employee handbook and procedure manuals, etc.

2. Attend and participate in assigned committee meetings, business meetings, mandatory training and continuing education programs.

3. Read memos, notices and communication books to keep abreast of changes occurring within the facility. Communicate information to AL staff members as appropriate.

4. Cooperate with other resident services and departments to maintain positive working relationships with all staff, residents, visitors and volunteers.

 

 

 

 

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

 

Maryland Licensed Registered Nurse and successful completion of the Maryland Board of Nursing approved Delegating Nurse/Case Manager training program. Must be knowledgeable of nursing and medical practices and procedures as well as laws, regulations, and guidelines that pertain to Assisted Living

 

COMPUTER SKILLS

 

Enter, adjust and retrieve data using a computer and keyboard.

 

LANGUAGE SKILLS

 

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of residents, visitors, or employees of organization.
